1 Duly filled up Admission form (on School Format).
2 Student Health History Form & Medical Certificate(on School Format).
3 Your Family (on School Format).
4 Questionnaire for Parents (on School Format).
5 Undertaking 1 regarding discipline (on School Format).
6 Undertaking 2 (on School Format).
7 Undertaking 3 (on School Format).
8 One recent passport size colour photograph .
9 One recent colour family photograph .
10 Original Birth Certificate , issued by the Municipal Corporation/ Any Govt. authority (the original will not be returned later on)..
11 Copy of Vaccination Card.
12 Two Address proofs (Electoral Identity Card/ Valid Passport/Latest paid Electricity Bill/ Rental agreement).
13 Copy of Aadhar Card of the child, mother & father.
14 Self-attested photocopy of SC/ST/OBC/Minority Certificate issued by DC Office(if applicable).
15 Self-attested photocopy of Child's Passport, if available .

PLEASE NOTE
1 Incomplete form will be rejected.
2 School transport once availed can not be discontinued in the mid of academic session.
3 School fee and transport fee are charged on quarterly basis..
4 Admission will be cancelled without any notice, if all the required documents are not submitted within 10 days of the date of admission.
